Used for the treatment of thyroid problems , Cytomel contains the active ingredient synthetic liothyronine sodium. Liothyronine, an L-isomer of triiodothyronine, is one of the forms of thyroid hormone.

Cytomel is a treatment option available for individuals with hypothyroidism, or an underactive thyroid. The medication can also be used in the treatment and prevention of goiters and also to diagnose other conditions that affect the thyroid.

Triiodothyronine and thyroxine are the two thyroid hormones that are produced by the body. When people have hypothyroidism, their thyroid is not producing enough of these hormones. Cytomel is a synthetic derivative to the hormone levothyroxine, which will help to restore the hormonal balance within the body.

Indication/Usage:
  • Cytomel, as indicated for the treatment of cretinism, myxedema and ordinary hypothyroidism, works as a replacement of the hormone normally synthesized by the thyroid gland.
  • Cytomel can be used instead of T4 for thyroid withdrawal patients.
  • Cytomel may be used in cases of thyroid cancer or Graves' disease as ablation therapy for the removal of any trace thyroid tissue.
  • Cytomel may be used in low thyroid functioning.
  • Cytomel may be used to treat or prevent goiters and sub-acute and chronic Hashimoto's thyroiditis.
  • Cytomel may also be used as a diagnostic agent in differentiating suspected mild hyperthyroidism or thyroid gland autonomy.
  • Cytomel may be used by those patients who are allergic to naturally desiccated thyroid or thyroid extract.

Dose, Administration and Dosage forms:

Cytomel tablets are available in 5 mcg, 25 mcg and 50 mcg once-a-day dosages to be administered orally. These tablets are round shaped and off-white in color. Inactive ingredients include starch, sucrose, calcium sulfate, gelatin, stearic acid and talc. The normal Cytomel dose is determined on a case-by-case basis and after careful observation of patient's response and laboratory findings. The starting dose depends mainly on the indication for use. Cytomel should be stored at normal room temperature between 15C and 30C (59 and 86F).

Generic Drug for Cytomel: Liothyronine

Mechanism of action: Cytomel is reported to be one of the most potent forms of thyroid hormone with a rapid onset of action. As a substitute for and/or supplement to the natural thyroid hormones, Cytomel works by increasing the basal metabolic rate (BMR). It also acts on protein synthesis and helps increase sensitivity towards catecholamine (like adrenaline) in the body. Cytomel actually enhances oxygen consumption in most human body tissues, thus increasing BMR and metabolism of carbohydrates, lipids and proteins.

Side-effects and safety profile: Cytomel may cause a number of side-effects with differing severity levels. Commonly reported non-serious side-effects associated with Cytomel include headaches, vomiting, diarrhea, increased bowel motility, upset stomach, stomach cramps, nervousness, weight loss, insomnia-related sleep problems, excessive sweating, enhanced appetite, fever, and altered heat sensitivity.

The common severe side-effects associated with Cytomel are tremor, arrhythmia, tachycardia, angina pectoris, congestive heart failure (CHF), irritability and mood changes, hyperthyroidism (with chronic usage), change in menstrual cycle pattern, and shock.

Rare but serious allergic reactions to Cytomel have also been reported.

Warnings/Contraindications:
  • Cytomel is contraindicated in patients with a history of cardiac issues like heart attack, chest pain, rapid heartbeat, pounding in the chest, etc.
  • Cytomel, categorized as a Pregnancy Category A drug, does not cross the placental barrier and is therefore safe for use during pregnancy.
  • Cytomel is excreted minimally via breast milk and may be used by nursing mothers with close monitoring by a physician.
  • Cytomel is contraindicated in patients who have been diagnosed with adrenal cortical insufficiency and thyrotoxicosis.
  • Physicians should guide patients to withdraw from Cytomel carefully and gradually over a six-week period.
